Eligibility: For those with a life threatening illness, and to those with a six month or less prognosis. Note: All referrals are directed to the Registered Nurse or Social Worker who will answer questions and follow through with appropriate services.
Service Description: Program that provides a full range of support services such as: medical care, pain and symptom management, home nurse visitation, care management, counseling and bereavement services for the patient and members of the family.

Background Checks: Performed on all employees and volunteers.
Comments:
*End of life services provided allowing individual to indicate health care and final arrangements
* Interpreters will be secured if available.
* Office Hours: Monday - Friday; 8:00 AM - 4:30 PM; Phone Answered: 24/7 (RN is always on call during non-office hours)
